About our 2025-2027 Action Teams
Health Dunn Right
The 2025–2027 Dunn County Community Health Improvement Plan (CHIP) is an action-focused roadmap that outlines how our rural community can work together to address the priority health issues identified by local residents through the 2024 Community Health Assessment.
The Health Dunn Right Coalition, along with partner organizations across multiple sectors, will use this community-guided plan to shape programs, policies, and collaborative efforts that strengthen health and well-being for all who live, work, and learn in Dunn County.
For 2025–2027, the following action teams will address Dunn County’s top health priorities:
